The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Caregiver (CG) Gertrudis Lazaro and discussed the purpose of the visit. CG called Administrator Diana Hernandez and Diana was available by phone to assist with the visit. The facility has six clients in care of which all were at day program or out in the community.

The facility is one story home with four resident bedrooms, two staff rooms, three bathrooms, kitchen, living room, dining room, laundry room, backyard and attached two car garage. The facility appears clean, safe and sanitary. LPA observed the kitchen to be clean and free of vermin. LPA observed a two day perishable and seven day nonperishable food supply on hand. LPA observed a fire extinguisher in the kitchen to be charged and with a purchase date of June 17, 2025. LPA observed the knives to be locked in a closet and made inaccessible to clients in care. LPA observed the centrally stored medication to be locked in a hall closet next to the kitchen and made inaccessible to clients in care. LPA observed a supply of extra linens in a hall cupboard for client use. LPA observed client bedrooms to have the required components and furnishings. LPA observed the bathrooms to be stocked with paper towels, toilet paper and non-slip mats. LPA tested the water to be between 103.6 and 104.3 degrees Fahrenheit. LPA observed the garage to have extra food storage and a locked cabinet where the toxins are stored making them inaccessible to clients in care. The backyard is free of debris and obstructions and has a seating area for client use. LPA and CG tested the carbon monoxide and smoke detectors and they were found to be operational.
