Facility Evaluation Report

LPA Haley attempted to conduct the required Annual Inspection.

When LPA arrived the door bell was rang and there was no answer.

LPA contacted the numbers on the facility profile. The primary number went straight to voicemail.

LPA then called the primary number on the profile and spoke with Faisal Rajpoot. According to Faisal he said his brother (Kamil Rajpoot) is the Licensee and he's his brothers partner.

When LPA explained the purpose of the call, Fasial said he is not ready for an inspection. LPA explained one the facility is licensed, the facility is subject to Annual Inspections.

LPA then asked about the construction taking place in the back of the facility. LPA asked for and email providing the following information:

Start and an estimated end dated of the construction project.

LPA then explained an attempt to conduct the required unannounced Annual Inspection will be attempted at a later date.

LPA request an email form the licensee following the phone conversation.
