Licensing Program Analyst (LPA) Hanna Gough made an unannounced case management visit to amend findings for complaint control #22-AS-20250915160323: which was delivered on September 17, 2025. LPA was greeted and granted entry by staff. LPA met with Caregiver (CG) Chris Moore and explained the purpose of the visit.
LPA explained to CG that the complaint findings of one Substantiated allegation is being amended to Unfounded.
An exit interview was conducted with CG Chris Moore and a copy of the amended findings and this report was provided to the facility.
