The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. Direct Support Staff Christian Moore contacted Administrator (AD) Marisa Hernandez via phone and LPA explained the reason for the visit and informed LPA that they would be arriving shortly. The facility has 6 clients. LPA observed one client at the facility and five are on outings.

The facility is a two story home with six client bedrooms, one staff room, two client bathrooms, one staff bathroom, living room, dining room, laundry room, kitchen, three car garage and a swimming pool. LPA observed the facility to be clean, safe, and sanitary. LPA observed the kitchen to be free of debris. LPA observed the two day perishable and seven day non-perishable food supply on hand. LPA observed fire extinguishers to be in the kitchen charged and serviced. LPA observed the sharps to be locked in a closet located in the hallway making them inaccessible to clients in care. LPA observed all client bedrooms to have the required components and furnishings. LPA observed the client bathrooms to have toilet paper, paper towels, and non-slip mats. LPA tested the water in the downstairs bathroom at 111.5 degrees Fahrenheit and the bathroom upstairs tested at 109.9 degrees Fahrenheit. LPA observed the medication to be stored in a locked cabinet located in the staff closet. LPA observed the emergency food and water supply in a closet located in the downstairs hallway. LPA observed the toxins and chemicals to be locked in a cabinet located in the garage making them inaccessible to clients in care. LPA observed clean linens in the upstairs hall cabinet for client use. LPA tested the smoke and carbon monoxide detectors and they were found to be operational. LPA observed the backyard to have a shaded seating area for client use. LPA observed the pool to have a 5ft mesh fence surrounding it on three sides and a brick wall on the fourth. The pool is locked and made inaccessible to clients in care. Continue on LIC 809-C
