Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a case management visit. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Marife Gironella and discussed the purpose of the visit.
LPA toured the facility and checked on Residents in care. LPA observed residents napping and watching television.
LPA discussed the fire clearance that was submitted to the Regional Office in January of 2026 with AD that is pending. AD informed LPA that during their inspection they were informed of changes and facility updates that needed to be made in order to pass the fire inspection and be granted a new fire clearance. AD informed LPA that the changes have been made and that they are now working on the documents that need to be sent to the Regional Office.
AD informed LPA that they will send all documents for a new fire clearance request along with an LIC200 to the Regional Office by close of business May 22, 2026.
An exit interview was conducted and a copy of this report was left at the facility.
