Facility Evaluation Report

On July 22, 2026, at 8AM, Licensing Program Analysts (LPAs) Eboni Bentley and Taylor Simerly conducted an unannounced Collateral visit in conjunction with complaint 22-AS-20260715164200. LPAs were greeted and granted entry by staff on duty and met with Licensee/Director Jane Le & Human Resource Director/Admin Assistant Thu Nguyen after explaining the purpose of the visit. The collateral visit was made in reference to ST. ANDREWS HOME FOR THE AGED (306005403). LPAs conducted client and staff interviews and obtained client records.

Based on the observations made during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations.

An exit interview was conducted with Human Resource Director/Admin Assistant Thu Nguyen and a copy of the report was provided.
