Facility Evaluation Report

Licensing Program Analyst (LPA) Jerome Haley conducted an unannounced case management visit to issue a citation for deficiencies discovered during the investigating of complaint control # 22-AS-20250522144453.

During the complaint investigation, it was discovered Client 1 (C1) has a history of making false statements and has accused staff of sexual assault in the past. According to Staff 1 (S1), sometime in March 2025, C1 made up allegations against two Del Sol Home female staff members. According to S1, the information was shared with Regional Center via email.

S1 was advised on the importance of always reporting incidents to the Regional Office in a timely manner, especially concerns or allegations made that are sexual in nature.

Violations are being cited per California Code of Regulations title 22.

An exit interview was conducted. A copy of this report and appeal rights were provided.
