Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by staff at 1pm and met with Joyce Zablan, Administrator.
The facility is a level 4e, single-story residential home with a fire clearance of three ambulatory and three non-ambulatory clients in rooms #1, #2 and #5. There are six bedrooms and two bathrooms. The facility has a census of five clients in care. At time of arrival, only one client remained in the home and the other four clients were at Adult Day Program (ADP).
During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in two of two client bathrooms, and testing auditory devices on all exits . The hot water temperature measured between 108.5 and 111.7 degrees Fahrenheit and all smoke detectors were operational . The fire extinguisher is charged and was serviced on September 16, 2025. The facility’s last fire drill was conducted on October 2, 2025 .
LPA inspected the facility food supply and observed the facility retained the two days perishable minimum and seven days non-perishable food on hand . LPA observed a variety of fruits such as apples, bananas and watermelon and the refrigerator was full of a variety of vegetables and meats. LPA obtained copies of the daily food menu and there was a variety of meals available for the clients. Clients were eating snacks upon arriving home from Day Program.
LPA toured the kitchen and observed all appliances were in working order, knives and sharps were locked
