Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 at 1pm. During today’s visit, LPA met with Lisa Roche, Licensee (LE).
The facility is a six bedroom, single story home with an approved fire clearance of six ambulatory residents of which six may be non-ambulatory and an approved hospice waiver for six. The facility currently has a census of five residents in care. Two of the six rooms are occupied by staff members who reside in the facility.
During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in two of two resident bathrooms, and testing auditory devices on all exits . The hot water temperature measured between 108.6 and 110.3 degrees Fahrenheit and all smoke and carbon monoxide detectors were operational . The fire extinguisher is charged and was serviced on June 20, 2025. The facility’s last fire drill was conducted on August 2, 2025.
LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand . Sharps and hazardous chemicals are secured. Kitchen appliances were in working order. The refrigerator had a variety of fruits and vegetables and the freezer was full since it was grocery day. There is an additional freezer in the garage; along with emergency supplies of water and food. The washer and dryer were also in the garage and operational.
LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. There are several First Aid Kits on the premises with the required elements.
