Facility Evaluation Report

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator Courtni English. LPA explained the reason for the visit. Facility is a single building with two levels. The second level is for the administration offices. The first level has 6 activity rooms, an art supply room and a large 10,000 square foot lunch and fitness area. Facility conducted it's last emergency drill on 10/20/2022. The fire alarm/sprinkler system was tested on 9/14/2022. LPA and Administrator toured the interior and exterior of the facility. LPA observed the see something say something poster (PUB 475), facility license and emergency disaster plan posted next to the facility entrance. LPA observed all activity rooms were provided with furniture in good repair, appropriate activity materials and kept free of tripping hazards. Two common bathrooms had water fixtures in good repair and provided with grab bars and adequate liquid soap and paper products. Hot water was measured at 109.0 degrees Fahrenheit. Common areas were clean and organized. The fitness area was provided with appropriate equipment and supervised by staff members. Break room had adequate storage space for clients in care and small kitchen appliances in working conditions. Cleaning supplies were kept in a locked in a storage room on the first level. Fire extinguishers were fully charged and mounted in common areas. No obstacles or hazards observed inside or outside of the facility. No deficiencies observed during the visit. No deficiencies are being cited as a result of this visit. LPA consulted with the Administrator concerning continued Covid-19 mitigation and reporting requirements. An exit interview was conducted and a copy of the report provided.
