Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Angelia Kruger, Administrator at 8am. LPA met with the Administrator and explained the purpose of the visit.
The facility is a level 2, six bedroom, three bathroom, single story home with an approved fire clearance of five ambulatory. The facility currently has a census of four clients in care. At the time of LPA's visit, all clients were at Adult Day Program.
During today’s visit, LPA toured the facility and tested smoke and carbon monoxide detectors. Detectors throughout the facility were operational. Fire extinguishers were charged and inspected on September 29, 2025. The facility's last fire drill was on January 8, 2026. LPA toured the kitchen and observed sharps and knives were secured under the sink. Chemicals are stored in a locked storage cabinet in the outdoor patio. LPA inspected the facility food supply and observed the refrigerators had more than a two-day supply of perishable items and seven days non-perishables on hand.
LPA walked into all clients bedrooms. Four of four bedrooms had the required furnishings and linens. LPA measured the hot water temperature in two of two client bathrooms. The hot water temperature measured between 114.6 to 117.8 degrees Fahrenheit. LPA toured the exterior of the property and observed the facility's emergency supplies with several jugs of water. The facility has a pet rabbit and pet chicken which clients enjoy playing with. There is an exterior shaded seating area for clients and pathways are free of obstructions. LPA observed many activities available for the clients.
