The inspector’s account

Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry at 1pm and met with Jill Efron, Operations Manager.

The facility is a single story industrial building with an approved fire clearance of forty non-ambulatory participants. The building space is shared with a church. Currently the census is twenty-five participants. Hours of operation are from 7:30am - 5pm. Most participants leave by 3pm.

During today’s visit, LPA observed participants in one large area with various activities in progress. LPA observed group discussions about jicama, coloring mandalas, or groups returning from an outdoor walk. Activities during the day included casino bingo.

While touring the building, LPA observed the First Aid Kit which had all of the required elements. Documentation of the most recent building fire alarm and carbon monoxide inspection, conducted on September 18, 2025, was obtained. There are fire extinguishers throughout the area which were recently inspected on February 13, 2026. The last quarterly fire drill was conducted on February 2, 2026. LPA tested the hot water temperature in the female bathroom. The water temperature tested 105.0 degrees Fahrenheit.

LPA observed a coffee station and two refrigerator/freezers. One refrigerator is for participants, the other for staff. Participants provide their own lunch but a food truck is available every Friday. Job coaches circulate . (Continued on LIC 809-C)
