Facility Evaluation Report

Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility to conduct a case management regarding an Unusual Incident Report received in our Regional Office. LPA was greeted and granted entry at 2pm. LPA met with Jill Efron, Program Director and Elisa Lopez, Program Supervisor and explained the purpose of the visit.

LPA toured the facility and conducted a health and safety check on all participants. At time of arrival clients were preparing to leave and were waiting for transportation. LPA spoke with clients regarding the activities they participated in and observed various groups of participants socializing while waiting to go home.

LPA obtained documentation from two of two client files which include: Identification Face Sheet, Physician's Reports, Individual Program Plans (IPP), Client Development and Evaluation Reports, (CDER) and Unusual Incident Reports.

Based on the observations made during today’s visit, the facility appears to be in compliance with Title 22 Division 6 of the California Code of Regulations, no deficiencies cited on this date. An exit interview was conducted with Jill Efron, PD and a copy of the report was given at the time of the visit.
