Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 at 8am . During today’s visit, LPA met with Julie Cooper, Administrator and explained the purpose of the visit.
The facility is a single story, five bedroom, two bathroom home with an approved fire clearance of five non-ambulatory and five bedridden. The facility currently has a census of five clients in care. Four staff members were present which include, a registered nurse and three direct support professionals.
While touring the facility, LPA observed the kitchen appliances were in working order, sharps and knives were secured in a locked drawer and chemicals/cleaning materials were secured underneath the sink. The refrigerator had a two-day perishable supply and seven-day non perishables are located in a storage closet. Washer and Dryer are also available
During today's visit LPA tested the hot water temperature in two of two bathrooms. The hot water temperature for both bathrooms was exactly 105.0 degrees Fahrenheit. The three fire extinguishers were charged and inspected on December 15, 2025. The facility's last fire drill was conducted on December 12, 2025 and a disaster drill was conducted on October 16, 2025. Auditory devices, smoke and carbon monoxide detectors were tested and were operational. LPA observed the Automated External Defibrillator (AED) and it is checked on a monthly basis and was inspected on January 2, 2026.. All staff are current with First Aid, Cardiopulmonary Resuscitation (CPR) and AED Training. First Aid Kits were observed throughout the facility and had the required elements with an American Red Cross Manual.
