Facility Evaluation Report

Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of interviewing Client #1 (C1) in connection to a complaint investigation at another licensed facility, complaint control #: 22-AS-20260603141501 . LPA was informed by Receptionist Wendy Clemente that C1 is absent from program today due to a personal appointment.

An exit interview was conducted with Case Managers Natalie Vialpando and Nicki Ngo, and a copy of this report was provided at exit via email.
