Facility Evaluation Report
This unannounced visit is being conducted by Licensing Program Analyst (LPA) Samer Haddadin for the purpose of performing the mandatory annual inspection. LPA was granted entry by Program Director (PD), Minaya Wright, along with Reginal Manager (RM) Teresa Crisan and explained the reason of the visit.
LPA along with PD and RM toured the exterior and interior of the facility and observed the following:
This is a large commercial facility which houses one day program. Facility has 2 large common areas, multiple small group rooms, multiple storage areas and offices, and 3 bathrooms. There is a side patio with shaded seating for the clients. The program operates between the hours of 8AM and 2PM. LPA observed 32 staff, and 77 clients present for this program. LPA observed clients and staff engaging in program activities throughout the facility. LPA inspected all rooms and areas of the facility.
Bathrooms: the bathrooms were clean; faucets and toilets were operational. Water temperature: tested at 113.9 degrees F in the client bathroom. Emergency Phone Numbers and Exit Plan: reviewed.
Food Service: this facility provides breakfast to qualified clients, and LPA inspected the food supply for breakfast. Carbon Monoxide, Smoke Detectors, Fire Extinguisher: inspected. Facility last emergency drill was conducted on March 27 th , 2025.
Appliances: microwave and refrigerator inspected. Knives: observed locked in the kitchen. Toxins: observed locked in the storage closets. Medication cabinet: observed to be locked. First-Aid Kit and Activity Supplies: observed and available.
