The inspector’s account

Licensing Program Analyst (LPA) Ruth Martinez conducted an unannounced annual required visit. LPA arrived at the facility was greeted by receptionist and granted entry. LPA met with Sarah Grover, Administrator and explained the nature of the visit.

At approximately 11:15am LPA was assisted on this day program tour by Sarah Grover, Administrator. LPA observed that the facility has designated rooms for different activities which include but are not limited to activity rooms, craft room, quiet room when clients need rest, a tv room for individual 1:1 care, and 3 restrooms. The facility serves snacks in the morning only and clients provide their own lunched during their day at the program. The kitchen area provided for food preparation is clean, and food available is sealed and dated. LPA tested the water temperature in the client’s restroom where hand washing is done, and measurements was 114.8 Fahrenheit Degrees at 11:32am. During the tour LPA did not observe any item accessible to clients which would pose a risk to their health and safety. Vehicle maintenance logs are maintained, vehicle registration is current. The facility maintains a locked location for chemicals, sharps and for medication. Evacuation plan, client’s rights, facility license, and emergency disaster plan and emergency exits are posted on the wall. LPA observe the fire safety maintained and sprinkles/smoke detectors are checked yearly and quarterly. Fire extinguishers are charge and mounted through out the facility. Fire disaster drills are conducted once a month. LPAs observed a covered outdoor patio area with seating for clients use. LPA reviewed medication storage and documentation; day program only administers midday medications for those clients that require it. During the inspection LPA reviewed 7 client records and 4 staff records.

Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations.

This report was reviewed with the Administrator and a copy of this report was provided to the facility.
