The inspector’s account

The complaint alleged that hazardous chemical odor was present at the facility on the day the complaint was made. During the visit, LPA Haddadin toured the interior areas of the facility, including common areas, activity areas, restrooms, and areas accessible to clients. LPA Haddadin did not smell nor observe any hazardous chemical odor, strong bleach odor, or unsafe conditions related to chemical use at the time of the visit.

LPA Haddadin observed that all chemicals and cleaning supplies were locked and secured in the maintenance room located in the back of the facility, which is inaccessible to clients. During interviews, four staff members denied the allegation and stated that cleaning products are used as part of the facility’s routine cleaning and sanitation practices and the cleaning takes place after hours and prior to clients’ arrival.

LPA Haddadin also interviewed four clients. All four clients denied the allegation and did not report feeling unsafe at the facility. The clients interviewed did not report experiencing illness, discomfort, injury, or respiratory issues related to a chemical odor at the facility.

Based on LPA Haddadin’s observations and interviews conducted there was insufficient evidence to support that staff failed to prevent the facility from having a hazardous chemical odor or that clients’ health and safety were placed in jeopardy.

Based on the information obtained, the preponderance of evidence standard was not met. Therefore, the allegation “Staff did not prevent the facility from a hazardous chemical odor” is found to be unsubstantiated.

An exit interview was conducted, and a copy of this report was provided to the Program Director, Minaya Wrights.
