The inspector’s account

Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 and met with Administrator (AD) Augustine Au. LPA explained the purpose of the visit.

The facility is a level 6, seven bedroom, three and a half bathroom, two story residence with a fire clearance of four ambulatory and two non-ambulatory clients. Currently there are five clients in care in which one client is non-ambulatory. At time of visit, LPA observed five of five clients waiting to leave for Day Programs.

During the visit LPA toured the kitchen and observed all appliances were in working order. A weekly menu was observed on the refrigerator and there were fresh fruit available for clients. The refrigerator had more than two days perishable food items and the pantry and freezer had more than seven days of non-perishable food on hand. The cleaning and hazardous chemicals are locked and stored beneath the kitchen sink. Knives and sharps were also secured under the kitchen sink. Emergency supplies were observed in a storage closet near dining room.

The smoke and carbon monoxide detectors were inspected and were operational. The fire extinguishers were charged and serviced on May 13, 2025. The fire drill was conducted on March 5, 2026. An evacuation chair is available upstairs in case of an emergency since once client bedroom is upstairs. LPA tested three of three hot water temperatures in client bathrooms. The hot water temperature ranged between 105.5 to 109.4 degrees Fahrenheit. Client bedrooms had the required furnishings and linens. The facility was odorless and clean. Bathrooms had non-skid mats and grab bars and covered trash bins. There were no chemicals or sharps in client bathrooms.
