Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 at 8am. During today’s visit, LPA met with Maxine Kniazeff, Assistant Administrator (AA),
The facility is a level 6, six bedroom, three bathroom residential home with an approved fire clearance of six ambulatory clients. The facility currently has a census of six clients in care. One client was attending Day Program at the time of the visit.
During the visit, LPA toured the facility and tested the fire and smoke detectors; which were operational. The kitchen appliances were in working order and sharps and chemicals were secured underneath the sink. The refrigerator had more than two-days perishable items and the facility had seven-days of non-perishable items on hand. Emergency supplies were observed in a storage cabinet with ample water. The fire extinguisher was charged and inspected on May 5, 2025 and the last fire drill was conducted on January 5, 2026.
LPA measured hot water temperatures in three of three client bathrooms. The temperature ranged from 105.2 to 109.1 degrees Fahrenheit. No odors were detected in client bedrooms and the required furnishings and linens were observed. LPA walked the exterior and observed a shaded seating area, both exterior gates had latches and worked properly and all window screens were repaired. The garage has exercise equipment for clients to use such as a treadmill and bicycles. There were no hazards or obstructions in outdoor pathways and ample space for activities.
