Facility Evaluation Report

Licensing Program Analyst (LPA) Rose Ruppert conducted an unannounced case management visit to follow up on an Incident Report received in our office on September 17, 2024. LPA was greeted and granted entry into the facility by Thien Luu, Administrator and explained the reason for the visit.

The purpose of our visit is follow-up on a bathroom remodel, which began on September 16, 2024, to convert a bathtub to a standing shower to accommodate the clients' access. LPA observed another bathroom which the four clients may use while the remodel is taking place. All families, as well as our Department and Regional Center,were notified of the temporary construction which is estimated to take two weeks.

During the tour, LPA observed a client asleep on the couch and another waiting by the door for transportation to Day Program. LPA spoke with Administrator Luu regarding estimated time of completion. LPA shared contact information, as well.

A copy of this report was given to the Administrator at time of exit.
