Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to amend a report given to the facility on October 7, 2025. LPA was greeted and granted entry by staff and met with Administrator, Leo Castaneda. LPA explained the purpose of the visit.
LPA amended the Unfounded report from October 7, 2025 to mark it as Confidential.
An exit interview was conducted and a copy of the amended report, with signatures, was provided to the facility.
