Facility Evaluation Report

Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Assistant Administrator (AA) Steven Martinez at 8am and explained the purpose of the visit.

The facility is a four bedroom residential home with three bathrooms. The facility has a fire clearance for four ambulatory of which one may be non-ambulatory in Room 4. The facility has a census of four clients; of which one was out with family and a second client was at Day Program. A third client was preparing to go to a Work Program meeting and the fourth client was working with Direct Care Staff in an activity.

During the visit LPA toured the kitchen area which was clean. Sharps and knives were secured; as well as cleaning supplies and laundry detergents. Fresh fruit were observed on countertops and all appliances were operational. The refrigerator had two-days of perishable items and the facility had seven-days of non-perishables. A weekly menu was observed in the front hallway and is created by a dietician.

LPA inspected two of three client bathrooms. All bathrooms had trash bins with covers, grab bars and non-skid flooring. The hot water temperatures ranged between 111.2 to 114.9 degrees Fahrenheit. LPA observed client bedrooms and bedrooms were clean and no odors were detected. The facility was 73 degrees and fire extinguishers were inspected on April 2, 2026. The facility's last fire drill was conducted on February 26 and March 29, 2026. The facility has a Automated External Defibrillator (AED) which was checked in February 2026. LPA inspected the smoke and carbon monoxide detectors, All alarms were working and operational.
