Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 at 8am. During today’s visit, LPA met with Augustine Au, Administrator.
The facility is a level 4i, six bedroom, three bathroom, single story home with an approved fire clearance of six ambulatory clients. The facility currently has a census of five clients in care. At time of arrival, one client had left for day program and two more were awaiting transport to attend day programs.
During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in three of three client bathrooms . The hot water temperature measured between 109.0 and 116.2 degrees Fahrenheit and all smoke and carbon monoxide detectors were operational . The fire extinguisher is charged and was serviced on May 5, 2025. The facility’s last fire drill was conducted on May 5, 2025 .
LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand with a variety of fruit and vegetables in a new refrigerator/freezer. All appliances were in working order, a menu and dietary orders were observed on the refrigerator and chemicals were secured under the sink.
LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. The First Aid Kit had all of the required elements; as well as a CPR Manual.
