Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 (S1). LPA met with Kathya Alpizar, Administrator (AD) and Thien Luu, Licensee (LE).
The facility is a single-story, four bedroom, three bathroom, residence with a fire clearance of four ambulatory clients. Currently there are three clients in care. At time of visit, one client was working at Adult Day Program and two clients were in their rooms playing video games.
During the visit LPA inspected the carbon monoxide and smoke detectors. Carbon monoxide and smoke detectors were operational. LPA observed the fire extinguisher was charged and inspected on October 22, 2025. The facility conducted its last fire drill on 12/31/2025. The temperature in the facility was comfortable at 74 degrees.
LPA toured the kitchen and observed the monthly menu on the refrigerator. Stove and kitchen appliances were in working order. The refrigerator/freezer had more than the two day perishable foods and seven days of non-perishables. Emergency water and food was observed in the garage. A washer and dryer were operational and chemicals were stored and locked in a cabinet. The sharps and knives were secured underneath the kitchen sink.
LPA observed client bedrooms and noted they had the required furnishings. Two of three clients were in their room playing video games.
