The inspector’s account

On May 7, 2025, Licensing Program Analyst (LPA) Samer Haddadin arrived at the facility to conduct the required annual inspection. LPA Haddadin was greeted and granted entry by Administrator (AD) Shelly Martinez, who holds a valid Administrator Certificate expiring on January 14, 2027.

LPA Haddadin and AD Martinez toured the facility, which is a single-story house containing four bedrooms (one designated for staff), two bathrooms, a living room, a kitchen, a dining room, a laundry room, and an attached two-car garage. During the tour, LPA Haddadin observed that the fireplace in the living room was screened. LPA Haddadin also noted puzzles and games for client recreation stored in the living room.

The facility appeared thoroughly clean, sanitary, organized, comfortable, and accommodating for all residents. Each resident's room contained the required elements, including a bed, a chair, closet space, and ample lighting. The restrooms were stocked with soap and paper towels, and handwashing instructions were displayed. The hot water temperature was measured between 105.4 and 107.2 degrees Fahrenheit. All sharps and knives were locked in a kitchen drawer, making them inaccessible to clients in care. Similarly, all chemicals were locked and inaccessible to clients in care.

LPA Haddadin noted that the facility maintained an emergency supply of food and water. The fire extinguishers were fully charged, as indicated by the arrow positioned within the green section of the meter. The extinguishers had been last serviced on April 2, 2025.

LPA Haddadin reviewed two client files and two staff files and found no deficiencies. The quarterly Emergency Drill record was also reviewed and indicated that the last drill was conducted on May 7, 2025.
