Complaint Investigation Report
S1 denied the allegation. The Administrator (AD) reported that they have never witnessed any such actions, and that no client’s or staff reported any staff threatening clients. Staff 2 (S2) and Staff 3 (S3) were interviewed, and they could not corroborate the allegation.
Client 2 (C2) Reported that they have never witnessed any staff speaking to clients in an inappropriate manner and have never witnessed any client having their personal items taken away from them. Client 1 (C1) reported that staff did not threaten to take away any of their personal items. Based on the evidence gathered the allegation, Staff is threatening client to take away personal items, is deemed unsubstantiated, although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur.
The investigation into the allegation, staff yelled at client revealed the following. It was alleged that Staff 1 (S1) yelled at Client 1 (C1). There was no additional information given regarding what S1 said to C1. It was reported that S1 raised their voice to C1. S1 denied the allegation. AD reported that they have never witnessed any yelling between staff members and clients in care. S2 and S3 were interviewed, and they could not corroborate the allegation.
C2 Reported that they have never witnessed any staff yelling at clients in care, and that they have never yelled at them. C1 reported that staff did not yell at them and that they were afraid that it was going to happen, however the allegation never took place. Based on the evidence gathered the allegation, Staff yelled at client, is deemed unsubstantiated, although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur. An exit interview was conducted and a copy of the report provided.
