Facility Evaluation Report

Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of interviewing Client #1 (C1) in connection to a complaint investigation at another licensed facility for complaint control #: 22-AS-20260603141501. LPA was greeted and granted entry by Care Provider Hasayn Bailey and explained the reason for the visit. Per Care Provider, C1 was at their personal appointment with staff. LPA conducted the interview via FaceTime. LPA obtained records for C1 which includes the Client Roster and Face Sheet.

An exit interview was conducted with Care Provider Hasayn Bailey, and a copy of this report was provided via email at exit.
