Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation for 2026. LPA was greeted and granted entry by Staff #1 (S1) at 1pm. LPA spoke with Administrator Adela Albu and explained the purpose of the visit via phone and met with Administrator Designee, Ciprian Albu. The Administrator inquired about the visit since an annual was conducted on April 24, 2026. LPA explained the April annual visit was for 2025. LPA also spoke with Administrator Designee (AD) regarding licensing fees and AD showed LPA fees were paid on July 14, 2026.
The facility is a seven bedroom, three bathroom single story residential home with an approved fire clearance for six non-ambulatory residents and an approved hospice waiver for three. Currently there are three residents in care and none of the residents receive hospice services.
During today's visit LPA toured the physical plant and inspected the carbon monoxide and smoke detectors. All carbon monoxide and smoke detectors were operational and an additional smoke detector will be purchased in the dining room. The fire extinguisher was charged and inspected on April 21, 2026 and the last fire drill was conducted on April 24, 2026. LPA toured the kitchen and observed chemicals and knives were secured and not accessible to residents. The refrigerator had two days of perishable food and the freezer, pantry and garage had seven days worth of non-perishable items and water. A washer and dryer were operational and medications were locked and secured. The facility temperature was 75 degrees Fahrenheit.
LPA tested hot water temperatures in three of three resident bathrooms. The hot water temperatures ranged from 105.6 to 107.4 degrees Fahrenheit. Resident bedrooms were clean and had the required furnishings and bed rail orders were on file.
