Facility Evaluation Report
Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a required/annual inspection. LPA knocked on the door and was met by a new tenant living at the residence. The new tenant stated that the residence was no longer being used as a facility and stated they had been living there since February 2025. LPA inquired if it would be okay to have a quick tour of the property to ensure that no clients were in care, tenant declined. LPA left business card with new tenant.
LPA then contacted Administrator (AD) Laura Rubio by phone at 10:05 a.m., AD stated the facility had been closed since October 2024 and stated she did not know who was currently occupying the property as they are not the home owner and their lease had since ended. LPA inquired why AD had not advised the Department of the facility closure and AD stated she was not aware the closure needed to reported. LPA advised AD to always inform the Department of any facility closure. AD stated they understood.
LPA thanked AD for their time and ended the call.
