Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced site visit to the facility for the purpose of amending Complaint Control Number 22-AS-20260311085816. Upon arrival, LPA met with Staff Mariam Esquivel, explained the reason for the visit, and conducted a health and safety check of the facility.
During the visit, LPA provided Mariam Esquivel with a copy of the amended report.
An exit interview was conducted, and a copy of this report was provided at the conclusion of the visit.
