The inspector’s account

On November 3, 2025, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced continued annual inspection from October 7, 2025. LPA met with staff Ana Alvarez and explained the purpose of the visit. Ana called Administrator Sharonda Espana via telephone and she was present via teleconference.

LPA toured the interior and exterior portions of the facility with Ana. The facility is an Adult Residential Facility (ARF) licensed for four ambulatory clients. It is a single-level structure. For this visit, there are a total of three clients in care.

There are a total of four client bedrooms, all of which are private client rooms. LPA toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Hardwired smoke detectors were tested and operational. Carbon monoxide detector was also tested and operational. There are a total of three bathrooms, two are for clients and one is for staff. Bathrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature in the bathrooms were measured to be between 111.7 and 113.7 degrees Fahrenheit.

Facility met the minimum two day perishable and seven day non-perishable food supplies. A bottle of mayonnaise had an expiration date of July 4, 2025. Sharp items and knives were locked in the medicine cabinet. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguisher was dated and tagged for the date of April 2, 2025. LPA observed the emergency disaster and evacuation plan, which is posted in the dining room. Facility had back-up emergency food and water supplies, located in the garage. LPA observed that First Aid Kit had all the required components. Continued on LIC 809-C.
