Facility Evaluation Report
On July 16, 2026, Licensing Program Analyst (LPA) Taylor Simerly and Licensing Program Manager (LPM) Alfonso Iniguez conducted an unannounced annual visit. LPA and LPM were greeted and granted entry by staff. LPA and LPM met with Martha Crawford, Administrator, with a valid administrator certificate from August 5, 2025 to August 4, 2027. LPA and LPM explained the purpose of today’s visit. The facility is licensed to serve (6) elderly adults ages 60 and above, of which (5) can be non-ambulatory. The facility has an approved hospice waiver for (1). Currently the facility has (2) residents and neither of them are currently on hospice.
This facility is a one storied home with seven bedrooms six of which are resident rooms, and one of which is a staff room. There are six bathrooms one of which is for staff and visitors, and five bathrooms that are for resident use only. There is a two car detached garage. There is a fully fenced pool with the fence measuring at 4 feet 11 inches.
LPA Simerly, LPM Iniguez and the executive director toured the physical plant. LPA and LPM inspected residents’ bedrooms and bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged from 108.8°F to 110 F, and a comfortable room temperature.
