Facility Evaluation Report
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit for the purpose of completing a required annual inspection. LPA met with Staff (S1) Charly Caoleng and discussed the purpose of the inspection. Five clients reside at this facility, three of whom were present while two were attending a day program. The facility is a one-story, six-bedroom, two-bathroom house with a detached garage currently used for storage. LPA reviewed medications and observed that they were properly labeled and stored in a manner inaccessible to clients in care. During the inspection of the bathrooms, LPA measured the hot water temperature at 112.6 degrees Fahrenheit. All bathrooms were observed to have a sufficient supply of soap, toilet paper, and towels. LPA confirmed that required department postings are displayed on the facility wall and that the facility remains within capacity limitations. There is a minimum of one week of non-perishable food and two days of perishable food available, with additional food storage maintained in a spare refrigerator and pantry located in the garage. Smoke detectors were tested and found to be operational. LPA toured the exterior of the facility and found passageways to be free of obstructions, and a shaded seating area is provided for client enjoyment. A fire extinguisher, with a last inspection date of February 2, 2026, is located in the kitchen. Fire drills are conducted quarterly, with the most recent drill performed on February 2, 2026. LPA reviewed three client records and three staff files, confirming that all required documentation is present and current. Based on the observations made during today’s visit, no deficiencies were cited. An exit interview was conducted and a copy of this report was provided to staff.
