The inspector’s account

Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 (S1) at 1pm. The Administrator was notified of the purpose of the visit and Administrative Designee, Luis DeJesus, was present during the visit.

The facility is a four client bedroom, one staff bedroom, two bathroom, level 3 , single story residential home with an approved fire clearance of four ambulatory and two non-ambulatory clients. The facility currently has a census of six clients in care.

During the visit, LPA toured the facility and inspected the carbon monoxide and smoke detectors. All detectors were operational. The fire extinguishers were charged and inspected on August 18, 2025. The last fire drill was conducted on January 24, 2026. LPA toured the kitchen which had more than the two-day required perishables and seven-day non-perishables on-hand. All appliances were operational. Knives and hazardous chemicals were locked and stored underneath the kitchen sink.

LPA inspected the hot water temperatures in two of two bathrooms. The hot water temperature for two of two bathrooms were both 107.4 degrees Fahrenheit. LPA toured the exterior of the property and there was a self-latching gate and ample room in the backyard for activities. The house will soon be painted so the shaded seating area was put away at this time. There were no hazards and obstructions in pathways.

LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. The First Aid Kit also had all the required elements.
