Facility Evaluation Report

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to complete the facility’s required annual inspection. Upon arrival, LPA Haddadin was greeted by Administrator (AD) Nikki Santiago, granted entry, and informed the AD of the purpose of the visit. The facility is a one-story home consisting of six client bedrooms, two bathrooms, a kitchen, dining room, living room, backyard, and an attached two-car garage. The garage is used for storage. LPA observed emergency supplies and water stored in the garage. The facility has four clients in care; two clients were present at the time of the visit, and two clients were attending a day program. LPA tested the smoke detectors and carbon monoxide detectors in the common areas and client bedrooms and found all devices operational. Client bedrooms were observed to contain the required furnishings, including appropriate furniture, bed linens, and adequate closet and/or drawer space to accommodate each client comfortably. Client bathrooms were inspected, and toilets and water faucets were operational. Grab bars were secure, and the shower area was observed to be free of mold and mildew. Water temperature was measured at 111.3 degrees Fahrenheit in the first bathroom and 113.5 degrees Fahrenheit in the second bathroom. Bath towels and personal hygiene supplies were adequately stocked at the time of the visit. Common areas were clean, free of hazards, and doorways were clear and unobstructed. LPA inspected the first aid kit and confirmed it contained required supplies, including bandages, tweezers, a thermometer, and scissors.
