The inspector’s account

Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 at 8am. During today’s visit, LPA met with Melissa Punzalan-Tyndall, Administrator .

The facility is a six bedroom two-and-a-half bath, level 4i single story home with an approved fire clearance of five ambulatory and one non-ambulatory. The facility currently has a census of six clients in care. Four clients had left for Day Program at 8am and two clients were picked up by Day Program transport at 9am.

During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in three of three client bathrooms, and testing auditory devices on all exits. The hot water temperature measured between 107.6 and 109.4 degrees Fahrenheit and all smoke detectors were operational . The fire extinguishers were charged and were serviced on April 3, 2025. The facility’s last fire drill was conducted on April 1, 2025 . Emergency disaster supplies and food were located in the garage.

LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. There was a variety of food and two fully stocked refrigerators/freezers. Knives and disinfectants were secured in locked cabinets. All appliances such as the stove, washer and dryer, were operational.
