The inspector’s account

LIcensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #`1 (S1). LPA met with Linda Punzalan, Administrator.

The facility is a seven bedroom, three bathroom, level 4g single story home with an approved fire clearance of f our ambulatory and two non-ambulatory clients; with a hospice waiver for two. The facility currently has a census of five clients in care.

During the visit, LPA toured the facility and tested all smoke detectors. Fire and carbon monoxide detectors were all operational. The fire extinguisher in the kitchen was charged and checked on April 3, 2025. The last facility fire drill was conducted on October 10, 2025. LPA visited five of five client bedrooms and observed bedrooms had the required furnishings. Three of three bathroom temperatures were tested and ranged from 111.2 to 112.1 degrees Fahrenheit.

The facility had sharps and knives secured in a locked drawer and cleaning supplies and chemicals locked beneath the kitchen sink. LPA observed the refrigerator was stocked with two days of perishables and seven-days of non-perishables in the pantry and garage. Appliances were in working order and a weekly menu was posted on the refrigerator. Additional emergency supplies and water were located in the garage with the washer and dryer.

LPA toured the exterior and there were no hazards obstructing pathways. A shaded area had seating as well as two treadmills for clients to use. There is sufficient room for outdoor activities.
