Facility Evaluation Report
Licensing Program Analyst (Samer Haddadin) conducted an unannounced visit today to complete a required annual inspection. Upon arrival, LPA met with Administrator Melissa Punzalan to discuss the purpose and scope of the evaluation. The facility is a single-story residence featuring six bedrooms and three bathrooms. An attached garage is utilized for general storage and the housing of emergency food and water supplies. At the time of the visit, there were six clients enrolled in care, all of whom were attending their respective day programs. A comprehensive tour of the interior and exterior was conducted. LPA and the Administrator tested the smoke and carbon monoxide detectors in both common areas and bedrooms, and all units were fully operational. Fire extinguishers were observed mounted in the kitchen and garage with the most recent inspection recorded on May 2, 2025. A review of the emergency drill logs indicated that the most recent drill was conducted on December 19, 2025. Resident bedrooms were equipped with the required furniture, bed linens, and sufficient closet and drawer space to accommodate each resident comfortably. All sharps and knives were observed to be stored securely in a locked kitchen area, remaining inaccessible to clients. Restroom toilets and water faucets were operational. Grab bars were found to be secure, and showers were free of mold or mildew. Water temperatures were measured at 111.2°F and 113.8°F. The backyard features a shaded seating area for resident use, and all emergency exits were inspected and found to be free of obstructions. LPA reviewed the files of two residents and two staff members, confirming that all required documentation was present and up to date. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22, Division 6 of the California Code of Regulations. An exit interview was conducted at the conclusion of the visit, and a copy of this report was provided to the Administrator.
