The inspector’s account

Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced case management visit for the purpose of following-up regarding a death report and conducting health and safety checks at the facility. LPA met with Administrator (AD) Dante Encarnacion and Staff Maria Engreso and explained the purpose of the inspection.

LPA, AD, and Staff Engreso conducted a tour of the inside and outside of the facility. The facility is a one-story apartment complex with 24 client bedrooms and 8 bathrooms. LPA observed select client bedrooms and observed they had the required furnishings. LPA observed client beds had linens and blankets. Water temperature in client bathrooms tested between 116.9-120.2 degrees Fahrenheit, and faucets and toilets were operational. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. Sharps and toxic chemicals were observed to be locked and inaccessible to clients in care. There is a fire extinguisher located in every facility hallway. Fire extinguishers were observed to be fully charged with service tags dated July 24, 2024. The facility has two courtyards with numerous shaded seating areas and no obstacles or hazards were observed.

LPA did not observe any immediate threats to the health or safety of clients in care.

Based on observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided at the end of the inspection.
