that needs medical intervention we call the necessary intervention. Interviews with 6 of 6 clients stated that they always get the help they need from staff, they have observed medical teams come to the facility and assist when a client needs it.
It is alleged facility is operating over capacity. Review of facility records on three separate visits revealed that facilities capacity is 40 clients at any given time and in all three visits it was verified that census for the facility was at capacity. Tour of the physical plant of the facility it was observed there were a total of 40 beds in the facility and no indications that there were clients outside of census of 40.
It is alleged that staff do not have adequate medication training. Records review revealed that facility roster reflects five medication technicians. Training records reflect that all five medication technicians on board have medication training on record as completed. Facility roster does reflect that all five staff are cross trained for multiple roles.
It is alleged that facility is dirty with pest infestation. LPA Alejandre toured the facility on February 2, 2022, and did not observe any pest, dropping or the facility to be dirty. LPA Martinez conducted a tour of the facility on June 22, 2026, and did not observe any pest issues or concerns with the facility being dirty. Facility was observed to be cleaned as well as kitchen. Interview with staff indicated that facility did not have a pest issue when the complaint visit was originally conducted. The facility did have a pest control vendor of October 2023 but not prior because there were no issues to be concerned about. Record review revealed facility had a pest control contract generated on October 11, 2023, for monthly services.
It is alleged that facility does not properly store or dispose of food. LPA Alejandre toured the facility on February 2, 2022, and LPA Martinez toured the facility on June 22, 2026, and both observed food being
