Facility Evaluation Report
Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator Mark Garciano and explained the reason for the visit. Facility has a capacity of 6 of which 2 can be non-ambulatory.
LPA and Administrator toured the facility. Facility is a single story home with 5 bedrooms, 3 bathrooms, 1 office, living room, eat in kitchen and a two car garage. Smoke detectors and carbon monoxide detectors tested operational. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed the 5 burner stove lights unassisted. The knives are kept locked in the closet along with the medications. LPA observed the fire extinguisher in the kitchen is fully charged. LPA observed all client rooms had the required furnishings and linens. The last emergency drill was conducted on September 2, 2024. LPA observed all 3 bathrooms are clean and operational. Hot water measured 107.6 degrees Fahrenheit. LPA observed extra linens in the hall closet. LPA toured the garage. The garage is used for storage and kept locked. LPA observed emergency food and water stored in the office. LPA toured the backyard. No bodies of water observed. LPA observed the sliding door in the living room has no sliding screen door. The exit gate is operational. There is a metal shed in the backyard. The shed is used to store supplies and kept locked. There is a shaded area with chairs for the clients to sit outside. No obstacles or hazards observed in the backyard. LPA reviewed 3 staff files, no discrepancies observed. All 3 staff are background cleared and associated to the facility. LPA reviewed 6 client records and P & I monies. No discrepancies observed. LPA reviewed 6 client medications, no discrepancies observed. LPA consulted with the Administrator concerning reporting requirements. LPA inspected the first aid kit. The first aid kit has all the required elements.
No deficiencies are being cited as a result of this visit. An exit interview was conducted and a copy of the report provided.
