Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 at 8am. During today’s visit, LPA met with Melba Santos, Assistant Administrator .
The facility is a level 2 , four bedroom and two bathroom residential home with an approved fire clearance of four ambulatory and two non-ambulatory clients in Room #3. Three of the bedrooms are for clients and one bedroom is for staff. The facility currently has a census of six clients in care. LPA observed four clients leaving for Adult Day Program, one client scheduled to work and another client chose to remain at home for the day.
During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in two of two client bathrooms, and testing auditory devices on all exits . The hot water temperature measured between 105.8 and 106.1 degrees Fahrenheit and all smoke and carbon monoxide detectors were operational . T he fire extinguisher is charged and was serviced on December 22, 2024. The facility’s last fire drill was conducted on August 9, 2025 .
LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand . Hazardous chemicals were secured under the sink and sharps were secured in a locked cabinet. There were plenty of fresh vegetables in the refrigerator and there are several storage areas for canned goods and emergency supplies. All kitchen and laundry appliances were in working order. LPA observed a posted menu and the "See Something, Say Something" Pub 475 poster.
