Facility Evaluation Report
On today's date Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by facility staff. LPA explained the purpose of the visit, and Administrator (AD) Laura Rubio was advised via telephone. AD arrived shortly afterwords and was able to assist LPA with the annual inspection. LPA began a tour of the facility and observed the following.
The facility is a two storied home equipped with six bedrooms; five of which are utilized for clients in care, three restrooms; two of which are utilized by clients and visitors, and an attached two car garage. LPA observed the kitchen area to be clean and free of any mildew and debris. LPA observed for the kitchen area to be equipped with a microwave, gas stove, and a refrigerator. All appeared to be in good repair and tested operational. LPA observed the facility to have a two day supply of perishable food and seven day supply of non-perishable food on hand. Additionally the facility has a sufficient amount of emergency water on hand.
LPA observed all client rooms to be large enough to walk about freely and to be large enough to accommodate all required furnishings including the following: A chest of drawers, a bed, clean linens in good repair; meaning no strains or tares, a chair, a reading lamp, and enough storage space to store personal belongings. LPA observed all client restrooms to be clean and free of any mildews and debris, LPA observed restrooms to have all required furnishings including the following: Grab bars, slip resistant floor matts, and a shower chair available if needed. LPA observed all faucets and toilets to be operational and in good repair. Hot water temperature tested between 108.3 and 108.9 degrees Fahrenheit.
