Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Policarpo Veliz, Administrative Designee . LPA explained the purpose of the visit and spoke with Licensee Laura Rubio and Administrator Felix Pesigan via phone.
The facility is a level 4i, two story, six bedroom home with an approved fire clearance of five non-ambulatory clients . The facility currently has a census of three clients in care. All clients and one staff member reside on the first floor and another staff member resides on the second story; which consists of a bedroom, bath and landing.
Upon entry at 8:30am, LPA observed two clients waiting for transportation to Day Program. One client had already left for Day Program. LPA observed client bedrooms which had the required furnishings and linens. LPA tested hot water temperature in two of two client bathrooms and the hot water temperature measured between 107.7 to 109.1 degrees Fahrenheit. All bathrooms were clean and non-skid mats were observed.
During today’s visit, LPA toured the facility and tested smoke detectors and all were operational . The fire extinguisher is charged and was serviced on April 9, 2025. The facility’s last fire drill was conducted on May 23, 2025 . LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand . There was a variety of food available and the pantry was accessible to clients. Emergency supplies were observed in the garage.
