Facility Evaluation Report
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced case management visit to the facility regarding a Special Incident Report (SIR) submitted to the Regional Office. Upon arrival, LPA Haddadin was greeted by staff member Ian Mendoza. Shortly thereafter, the Administrator, Louise Igisaiar, arrived and met with LPA Haddadin.
The SIR indicated that Client 1 passed away at Anaheim Regional Medical Center due to cardiac arrest. Facility records further documented that the client had a medical history of dyspepsia and hypertension.
No citations were issued at this time. An exit interview was conducted, and a copy of this report was provided to the facility.
