Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of interviewing Client #1 (C1) in connection to Complaint Control #: 22-AS-20230721111639 which unrelated to this facility.
During today's visit, LPA met with Program Director Karen Errington and explained the reason for the visit. LPA interviewed C1 and one staff during the visit.
An exit interview was conducted with Program Director Karen Errington, and a copy of this report along with the LIC811 were provided at the end of the visit.
