Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert attempted to make an unannounced visit for an Annual Required Evaluation at 8am. LPA knocked on front door and an unknown tenant opened the door and denied LPA entry.
LPA called the facility phone contact on file and spoke with Licensee's (LE) sister. The sister stated the licensee was traveling and unavailable to speak with LPA. Tenant asked to speak with the LE's sister to inform her that LPA would be denied entry and tenant closed the front door. LE did not have control of property. LPA stated to LE's sister that entry was required and that denial of entry would revoke the license. LPA will attempt to return at a later date to conduct the annual required evaluation.
Civil penalties will be given for: denial of entry and for an unknown tenant that is not background cleared. Both deficiencies are repeat violations from a Department visit on April 15, 2024.
The following deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
An exit interview was not conducted but a copy of this report will be mailed to the licensee address on file, via certified mail; along with a copy of the LIC 809-D, LIC 421FCs and Appeal Rights.
