Facility Evaluation Report
Licensing Program Analysts (LPAs) Rose Ruppert and Joseph Alejandre made an unannounced case management to conduct a health and safety visit to ensure there are no residents residing in the facility that require care and supervision. LPAs were assisted on the visit by the Long Term Care Ombudsman (LTCO) and a Vietnamese speaking Adult Protective Services (APS) Social Worker.
LPAs, LTCO and APS were approached by Administrator (AD), Thanh Trieu. LPA Ruppert explained the purpose of the visit and AD began to yell at LPA; stating to, "Stop bothering him" and left the facility.
LPAs, LTCO and APS toured the facility and observed there were no residents present. Resident #3 (R3), who is independent, was not at the facility but R3's belongings remain in their room and R3's vehicle was parked in the driveway. Residents #1 and #2 were not present at the facility and there were no personal effects remaining in their bedrooms. No immediate health and safety concerns observed during the visit.
An exit interview was not conducted and a copy of the report was left in the facility.
