The inspector’s account

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual inspection at the facility on February 4, 2026. Upon arrival, LPA Haddadin met with Care Staff Chona Padilla to explain the purpose and scope of the visit. The facility is a single-story residence consisting of five bedrooms and three bathrooms, with an attached garage utilized for locked storage. At the time of the inspection, two of the four resident clients were present, while the remaining two were attending a day program.

The physical plant tour commenced with an inspection of the interior and exterior premises. During the walkthrough, the LPA observed that two of the five bedrooms are currently being used for storage. These rooms were heavily obstructed by unorganized piles of boxes and footwear, preventing a thorough inspection of the living space. The volume of stored merchandise creates a significant safety hazard and negatively impacts the resident environment. Due to the lack of accessible floor space and generally unsafe conditions, the LPA was unable to effectively evaluate these areas.

In the accessible areas of the home, the LPA confirmed a clean supply of linens and sufficient lighting in the active client bedrooms to ensure safety and comfort. Smoke detectors were tested and found to be fully operational. The facility maintains the required food supply, including a minimum of one week of non-perishable goods and two days of perishable items. Medications were found to be centrally stored in a locked cabinet within the dining room; all medications were properly labeled and remained inaccessible to clients. {***CONTINUE***}
